Form submissions are transmitted via encrypted email to our practice inbox at [email protected]. We do not store form submissions in a database on our website. Once received, your information is handled in accordance with standard professional and HIPAA-aligned practices for healthcare communications. Please note that the initial contact form is not a HIPAA-covered channel — please do not submit protected health information (PHI) through this form.
4. Google Analytics
This website uses Google Analytics 4 (GA4) to understand how visitors interact with our site (e.g., which pages are visited, how long visitors stay). Google Analytics collects anonymized data such as browser type, device type, and general geographic region. It does not collect your name, email address, or any information you enter into forms.
